Endoscopic Sleeve Gastroplasty (ESG)
Endoscopic Sleeve Gastroplasty (ESG) is a minimally invasive, incisionless weight loss procedure. It reduces the size of the stomach using internal sutures, performed through the mouth using an endoscope. No part of the stomach is removed. The procedure helps patients feel full faster and for longer periods, leading to reduced calorie intake and sustainable weight loss.
How is the procedure performed?
- The patient is placed under general anesthesia.
- A flexible tube called an endoscope is inserted through the mouth into the stomach.
- Using a special suturing device, the doctor places a series of stitches inside the
stomach, reshaping it into a sleeve-like, tube shape (similar to a banana). - The stomach size is reduced by approximately 70%.
- No incisions are made on the abdomen – the entire procedure is done internally.
- The procedure takes about 45-60 min.
- Patients typically stay in the day care unit for 2-3 hours, then are free to go home.
Results:
- Patients usually lose about 15–20% of their total body weight over 6–12 months.
- Results vary depending on diet, exercise, and lifestyle adherence.
- Studies show weight loss can be maintained long term if healthy habits are sustained.
Compared to Surgical Sleeve Gastrectomy, the Endoscopic Sleeve Gastroplasty procedure does not require stomach resection and leaves no scars, as it is performed through the mouth. Recovery time is shorter – usually 2–5 days, compared to 2–4 weeks after surgical intervention. The procedure is theoretically reversible, with a lower risk of complications and minimal hormonal changes, although the weight loss effect is slightly less than after surgical sleeve gastrectomy.
This procedure is suitable for patients who are overweight or obese (ideal body mass index for candidates is 30–38), who do not wish to undergo or are not eligible for surgical operations, have not achieved sufficient results with diet and exercise alone, and are ready to change their eating habits and lifestyle in the long term.
Risks and Side Effects:
- Temporary bloating, nausea, or abdominal discomfort.
- Rare complications: bleeding, infection, or suture loosening.
- Less hormonal impact compared to surgical options.
- Weight loss depends heavily on patient compliance with diet and behavior changes.
Preparation:
One week before the procedure, patients must follow a liquid-only diet.
Post-procedure::
- First 4 weeks: liquid diet only.
- Gradual transition to soft, then regular food by 4–6 weeks.
- Small portion sizes and regular meals are essential.
- Ongoing support from nutritionists, psychologists, and physical activity coaches is highly recommended.
- Lifestyle change is crucial for long-term success.
